Listing Information:
We are certified by the state of Minnesota Department of Human Services to provide many human and social services to eligible Minnesotans who have Medical Assistance (Medicaid) and who need services. Among others, we provide Relocation Services Coordination to assist people living in qualified institutions to find housing and services in the community. We also provide Housing Stabilization Services to assist eligible individuals who are homeless, at risk of homelessness, living in a facility, or who are 65 years and over and need housing to find housing, connect them with needed services and resources, and help them maintain stable housing. We equally provide Comprehensive Home Care.
