Jay Blake, DNP is a Nurse Practitioner who specializes in dermatologic diseases and therapy. He was born in Saint Louis, and raised in Phoenix. He received his Master of Science in Nursing degree in 2010 from Yale University, followed by his Doctorate in Nursing Practice from Northeastern University in Boston. Mr. Blake completed his dermatology practitioner post-graduate training at the prestigious Lahey Clinic in Burlington, MA in 2011. Prior to his dermatology practice, he monitored clinical trial research for Genentech, Inc. in San Francisco. Mr. Blake is certified by the California Board of Registration in Nursing, and the American Nurses Credentialing Center. He enjoys spending time with his wife and their two dogs, and surfing whenever possible (with lots of sun protection).
